Zusammenfassung
In den vorhergehenden Beiträgen sind die beiden distinkten Perioden der ventrikulären Arrhythmien während der ersten 30 Minuten nach akutem Koronarverschluß schon eingehend beschrieben worden. Dies trifft sowohl für die Herzen von Hunden als auch von Schweinen zu. Wir haben allen Grund, dies auch für den Menschen als gleichrelevant anzusehen. Diese frühe Arrhythmiephase nach akutem Verschluß der LAD an 65 Hunden ist noch einmal in Abb. 1 demonstriert. Kammerflimmern und ventrikuläre Arrhythmien sind bimodal verteilt in zwei typischerweise voneinander getrennte Subarrhythmiephasen Ia und Ib (15). Gleichzeitig steigt die extrazelluläre myokardiale Kaliumaktivität ([K+]e) — im Ischämiegebiet epikardial gemessen — während der ersten Minuten schnell an und erreicht noch während der Ia-Phase ein Plateau, das während der gesamten Ib-Phase persistiert (8, 9, 17). Die schnelle Zunahme der [K+]e während akuter Myokardischämie korreliert mit ausgeprägten inhomogenen elektrophysiologischen Veränderungen, die stets mit dem Auftreten früher Arrhythmien verbunden sind (3, 10).
