Summary
The effect of early antibiotic administration (3−4 × 0.5 g Imipenem/Cilastatin), and maximal ICU therapy in 86 patients with proven necrotising pancreatitis was analyzed. In cases of clinical sepsis, patients underwent fine-needle aspiration (FNA) followed by necrosectomy and postoperative continuous lavage if positive for germs. Twenty-nine patients (34%) developed infected pancreatic necrosis after a mean of 21 days, while 57 cases were sterile. In 78%, the surgical concept was successful with a single operation; only 6 cases (22%) needed relaparotomy. Total mortality was 9.3% (8/86). Indication for surgery in severe acute pancreatitis should be postponed (3rd−4th week with good demarcation of the necrosis) and is solely given in infected pancreatic necrosis.
Zusammenfassung
Prospektiv wurde der Effekt einer frühen Antibiotikatherapie (3−4 × 0,5 g Imipenem/Cilastatin) und maximalen Intensivbehandlung bei 86 Patienten mit nachgewiesener nekrotisierender Pankreatitis untersucht. Bei Zeichen einer klinischen Sepsis erfolgte eine Feinnadelaspiration (FNA) und bei Keimnachweis die Nekrosektomie mit postop. geschlossener Lavage. Im Verlauf entwickelten 29 Patienten (34%) nach im Mittel 21 Tagen eine Infektion der Nekrosen, während 57 Fälle steril blieben. Das operative Konzept war in 78% der Fälle als alleinige Massnahme erfolgreich; lediglich 6 Patienten (22%) mussten relaparotomiert werden. Die Gesamtletalität lag bei 9,3% (8/86). Die Indikation zur Operation bei schwerer akuter Pankreatitis sollte spät (3.–4. Woche mit guter Demarkation der Nekrosen) und nur bei Nekroseinfekt gestellt werden.
