Zusammenfassung
Die Prognose des Patienten und das therapeutische Konzept (Operation, Radiotherapie oder Chemotherapie) werden beim Bronchialkarzinom von dessen histologischem Typ und seiner Ausdehnung wesentlich bestimmt. Die Masse des Tumors dokumentiert sich hauptsächlich in seiner anatomischen Ausbreitung. Zusätzlich liefern die Aktivitäten verschiedener Enzyme (z.B. Laktatdehydrogenase, neuronenspezifische Enolase), Hormone (z.B. ACTH, Parathormon, Kalzitonin) oder der Nachweis tumorspezifischer Antigene (z.B. CEA) Hinweis auf die Tumorausdehnung. Mit diesen Parametern ist es möglich, aus prognostischer Sicht Subgruppen von kleinzelligen Bronchialkarzinomen zu charakterisieren. Dies mag für klinische Studien hinsichtlich einer Stratifikation von Bedeutung sein, jedoch ersetzen diese Daten nicht die Parameter eines anatomischen Stagings (Stahel et al. 1989). Von prognostischer Bedeutung sind außerdem die Tumorverdopplungszeit, der Nachweis eines Gefäßeinbruches, die Anzahl von Fernmetastasen, das Vorhandensein klinischer Tumorsymptome, ein Gewichtsverlust von 10% und mehr sowie der Leistungsindex, das Alter, das Geschlecht und die psychischen Reserven des Patienten (s. Kap. 1.4).
