The Red Code: Management of Bleeding During or After Cleft Palate Surgery

Abstract

Introduction: Bleeding, during perioperative and postoperative period is not only frustrating for the surgeon but may also lead to life-threatening condition for the patient, if the bleeding is not stopped in time. Sometimes if uncontrolled or not managed adequately, it can lead to death of patient. It is a worldwide problem and may become more dangerous in pediatric patients and more specifically cleft surgery patient postoperatively.

Our algorithm is very helpful in the management and prevention of postoperative bleeding in cleft patients. We have named it as “Red Code Algorithm,” for pre-, intra-, and postoperative management of cleft patients. We have used this algorithm during cleft missions organized by Operation Smile in Ecuador, Peru, Paraguay, Nicaragua, and Morocco. We applied this algorithm in 864 primary and secondary cleft lip and palate surgeries, performed during free surgical cleft missions organized by Operation Smile in these countries for 1 year. Forty-five patients (5.3%) presented with more bleeding and two cases (0.23%) required surgical re-intervention to control bleeding. Before we applied this algorithm, 1243 Cleft surgeries were performed in these countries by the same group of surgeons. One hundred and thirteen cases (9.1%) presented with more bleeding, and 37 cases (2.97%) required surgical re-intervention to achieve hemostasis control. Our algorithm has reduced the incidence of postoperative bleeding in all types of cleft surgeries.
