The Speech Score: A New Concept for Evaluation of Functional Result after Cleft Patient

Abstract

The Alvarez Speech Score is presented as a measurement tool to evaluate the quality of speech in patients of cleft lip and palate, before and after surgical treatments and speech and language therapy.

This Scoring system was broadly applied in Ecuador, Peru, Paraguay, and Nicaragua, by 27 specialists (who were first trained in the score application); audio recordings of the corresponding words at each level of Speech articulation were submitted for analysis.

The numerical scale for each patient has a total score of 15 points. Each surgeon will listen to the patient’s voice and assign the score. If two or three surgeons differ in their score assessment, then their scores are added and an average is calculated for each individual patient.

The Alvarez Score system has proved to be an easily applicable tool, which is reproducible, provides useful parameters for the evaluation of surgical results, and gauges the competency level and technical skills of the cleft surgeons. It can detect failures and most importantly guide for a solution toward technical improvement.
