Sommario
La tiroidite di Hashimoto (TH) è la malattia endocrina di più frequente osservazione, caratterizzata da una forte aggregazione familiare, dalla predilezione per il genere femminile e da un processo autoimmune con anticorpi antitiroide circolanti e, nella forma più completa, con un’infiltrazione linfocitaria della tiroide che causa ipotiroidismo e conferisce un tipico aspetto ipoecogeno della ghiandola all’ecografia. La diagnosi di TH viene spesso formulata in modo improprio in pazienti con anticorpi antitiroide ma senza un’infiltrazione linfocitaria diffusa della ghiandola e non destinati a sviluppare ipotiroidismo. In questa rassegna proponiamo che, accanto alla presenza di anticorpi antitiroide circolanti e allo stato funzionale di ipotiroidismo, l’aspetto ipoecogeno della ghiandola all’ecografia tiroidea sia incluso tra i criteri diagnostici “maggiori”, mentre l’aggregazione familiare e l’associazione con altre malattie autoimmuni costituiscano i criteri “minori”.
