Abstract
An original method of CT measurement of the lateralization of the humeral intertubercular groove is described based on geometric construction following Thales theorem. A study of intra- and interobserver reproducibility was done of this measurement and humeral retroversion on 32 healthy volunteers. The results show good reproducibility of these measurements. The average value of humeral retroversion was lower than the average values found in the literature: 11.71° on average on the dominant side and 7.03° on average on the non-dominant side with a large spread of values. The reasons for these differences are discussed. The average values of lateralization of the intertubercular groove were 114.97° on the dominant side and 121.9° on the non-dominant side. These CT measurements are important to consider in the pathophysiology of chronic anterior instability of the shoulder.
Résumé
Ce travail expose une méthode originale de mesure tomodensitométrique de la latéralisation du sillon intertuberculaire huméral, s'appuyant sur une construction géométrique à partir du théorème de Thalès. Nous avons par ailleurs réalisé sur 32 témoins sains une étude de reproductibilité intra- et inter-observateur de cette mesure et de la mesure de rétroversion humérale. Les résultats montrent une bonne reproductibilité des mesures. La valeur moyenne de rétroversion humérale était inférieure aux valeurs moyennes trouvées dans la littérature, de l'ordre de 11,71° en moyenne du côté dominant et de 7,03° en moyenne du côté dominé, avec une large dispersion des valeurs. Les raisons de cette différence sont discutées. Les valeurs moyennes de latéralisation du sillon intertuberculaire étaient de 114,97° du côté dominant et de 121,9° du côté dominé. Ces mesures tomodensitométriques sont intéressantes à considérer dans la physiopathologie de l'instabilité antérieure chronique de l'épaule.
