Zusammenfassung
Fragestellung. Die perkutane Vertebroplastie mit Polymethylmethacrylat ermöglicht die minimal-invasive Stabilisierung osteoporotischer Wirbelfrakturen. Eine Frakturreposition ist jedoch nicht realisierbar und bei Berstungsfrakturen besteht ein erhöhtes Risiko von unkontrolliertem epiduralen Zementaustritt. Dagegen erlaubt die Kyphoplastie eine altersabhängige Frakturreposition und bietet ein erweitertes Anwendungsspektrum für offene Zugänge mit der Möglichkeit der spinalen Dekompression und Augmentation bei inkompletten Berstungsfrakturen.
Methodik. Bei der Kyphoplastie wird ein kontrastmittelgefüllter Ballon im Wirbelkörper aufgedehnt, bis eine Kaverne entsteht. In Abhängigkeit vom Frakturalter wird hierbei eine Reposition erreicht. Die Augmentation wird mit hoch viskösem Polymethylmethacrylat unter geringem Druck durchgeführt. Bei neuraler Kompressionssymptomatik wird eine interlaminäre Dekompression des Spinalkanals mit Kyphoplastie über die Hinterwand durchgeführt. Bei ventralen Wirbelsäuleneingriffen besteht die Möglichkeit der Kyphoplastie ohne Zugangserweiterung.
Ergebnisse. Wirbelaugmentationen durch Kyphoplastie wurden perkutan, interlaminär und ventral bei inkompletten Berstungsfrakturen durchgeführt. Vier representative Kasuistiken aus einem Gesamtkollektiv von 120 Augmentationen werden vorgestellt.
Schlussfolgerungen. Die perkutane Kyphoplastie, ergänzt durch offene Zugänge, ermöglicht die Augmentation bei osteoporotischen inkompletten Berstungsfrakturen.
Abstract
Objectives. Percutaneous vertebroplasty with polymethylmethacrylate allows minimally invasive stabilization of osteoporotic vertebral fractures. Fracture reduction is, however, not possible and the risk of uncontrolled epidural cement leakage with burst fractures is increased. Kyphoplasty, in contrast, allows a degree of fracture reduction and provides an extended spectrum of indications through open approaches, which enable spinal decompression and augmentation of incomplete burst fractures.
Methods. In kyphoplasty a contrast-filled balloon is inflated in the vertebra until a cavern is created. A degree of reposition may be achieved depending on fracture age. Augmentation is performed with high-viscosity polymethylmethacrylate under low pressure. In cases of neural compression, interlaminary spinal decompression and kyphoplasty through the posterior wall is performed. With anterior spinal procedures, kyphoplasty can be performed without extending the approach.
Results. Vertebral augmentation was performed by percutaneous, interlaminary, and anterior approaches for incomplete burst fractures. Four representative cases are presented from a collective of 120 augmentations.
Conclusions. Percutaneous kyphoplasty, supplemented by open approaches, enables augmentation of osteoporotic incomplete burst fractures.
