Zusammenfassung
Hintergrund
Im Rahmen eines Pilotprojekts wurde der Umfang des deutschen Neugeborenenscreenings (NGS) um 14 Erkrankungen erweitert. Die bisher untersuchten Probenzahlen erlauben eine Zwischenbilanz für (a) Hypertyrosinämie Typ I (TYR I) und (b) Propionacidämie, Methylmalonacidurie sowie Vitamin-B12-abhängige Störungen (PA/MMA/B12).
Material und Methode
a) Succinylaceton (anstelle des unspezifischen Tyrosins) als Markermetabolit für TYR I wird zum Hydrazon derivatisiert und gemeinsam mit Aminosäuren und Acylcarnitinen mithilfe der Elektrospray-Ionisation-Tandem-Massenspektrometrie gemessen. b) Bei Verdacht auf PA/MMA/B12 wegen erhöhter Werte für Propionylcarnitin (C3) werden in einer ergänzenden Zweitmessung („second-tier test“) aus derselben Trockenblutkarte Methylmalonsäure (MMS) und 3-OH-Propionsäure (3-OH-PS) mithilfe der Flüssigchromatographie mit Tandem-Massenspektrometrie-Kopplung quantitativ bestimmt.
Ergebnisse
a) Unter 480.000 Neugeborenen wurden 3 mit TYR I identifiziert. Falsch-positive bzw. falsch-negative Resultate sind bisher nicht bekannt. b) Es wiesen 503 von 300.000 Kindern eine erhöhte Konzentration von C3 auf. Unter 35 auffälligen ergänzenden Zweitmessungen ließen sich 3 Patienten mit PA, 3 mit MMA, 3 mit kongenitalen und 7 mit maternalen Vitamin-B12-Störungen konfirmieren. Das Vorgehen ermöglicht eine Senkung der „Recall“-Rate von 0,17 auf 0,01 % mit Erhöhung des positiven Vorhersagewerts von 3,2 auf 45,7 %.
Schlussfolgerung
Nach Einführung der optimierten Vorgehensweise wird die Aufnahme von TYR I sowie PA/MMA/B12 in das NGS-Programm empfohlen: a) Für TYR I sind eine hohe Sensitivität und Spezifität bei relativ geringem Mehraufwand für Personal/Material gegeben. b) Für PA/MMA/B12 erlaubt das beschriebene Vorgehen beim Screening auf C3-Erhöhung eine sensitive Erfassung mit signifikanter Senkung der falsch-positiven Rate.
Abstract
Background
In a pilot study the extent of screening of newborns in Germany was increased by 14 diseases. The data collected so far allow conclusions to be drawn for (a) tyrosinemia type I (TYR I) and (b) propionic acidemia, methylmalonic aciduria and vitamin B12 related disorders (PA/MMA/B12).
Material and methods
(a) Succinylacetone (instead of the unspecific tyrosine) as the diagnostic metabolite marker for TYR I is derivatized to hydrazone and simultaneously analyzed with amino acids and acylcarnitines by electrospray ionization tandem mass spectrometry. (b) In newborns suspected of having PA/MMA/B12 due to elevated concentrations of propionylcarnitine (C3), methylmalonic acid (MMS) and 3-OH-propionic acid (3-OH-PS) were quantified in a second tier testing using the identical filter card by liquid chromatography tandem mass spectrometry.
Results
(a) Out of 480,000 newborns 3 were identified as having TYR I. False positive and false negative results are not known so far. (b) Out of 300,000 children 503 showed elevated C3 levels. Among 35 pathological second tier tests 3 newborns were shown to have PA, 3 MMA, 3 congenital vitamin B12 disorders and 7 maternal vitamin B12 disorders. This approach results in a drop in the recall rate from 0.17 % to 0.01 % with an increase in the positive predictive value from 3.2 % to 45.7 %.
Conclusion
After implementation of the optimized approach, the inclusion of TYR I and PA/MMA/B12 in the screening program is recommended. (a) With respect to TYR I there is both high sensitivity and specificity combined with a relatively low incremental effort and cost increase. (b) With respect to PA/MMA/B12 the approach when screening for elevated C3 levels allows a sensitive identification with a significant reduction in false positive results.
