Parastomal hernias

Abstract

Parastomal hernia is a common complication of stoma formation, having an incidence of about 10%. As with many other ostomy-associated complications, most parastomal hernias are related to inadequate preoperative planning or technical errors. Most of these hernias should be managed conservatively; only 10–20% of patients eventually require operative intervention. When an operation is indicated, relocating the stoma generally yields better long-term results than does an attempt at local repair of the hernia.

La hernie parastomiale est une complication fréquente des stomies; son incidence se situe á 10% environ. Comme dans d'autres complications associées avec la confection d'une stomie, la plupart des hernies parastomiales sont en rapport avec une insuffisance de préparation ou des erreurs techniques. La plupart de ces hernies devraient être traitées de façon conservatrice: seulement 10 à 20% de ces patients nécessitent une réintervention. Lorsqu'indiquée, une relocation de la stomie est généralement suivie d'un meilleur résultat à distance que la réparation locale.

Resumen

La hernia paraestomal es una hernia incisional que se desarrolla en la vecindad de una colostomía o de una ileostomía, y es una de las más frecuentes complicaciones de la construcción de un estoma, con una incidencia de alrededor de 10%. Tal como ocurre con muchas otras complicaciones de las ostomías, la mayoría de las hernias paraestomales aparecen relacionadas con una planeación preoperatoria inadecuada o con errores técnicos.

La mayoría de estas hernias pueden ser manejadas en forma conservadora, y sólo 10–20 porciento de los pacientes eventualmente requieren intervención operatoria. Cuando la operación llega a ser indicada, la reubicación del estoma generalmente da mejores resultados que los intentos por reparar localmente la hernia.
