Abstract
The surgical care for patients with cleft palate with or without cleft lip has evolved over the past decades. The challenge in the modern cleft palate repair is no longer successful palatal closure but the achievement of an optimal speech outcome while minimizing deleterious effects on the maxillary development. Current palatoplasty techniques are resulting from lessons learned of many surgeons using seminal surgical principles. This chapter critically addresses the controversial issues in cleft palate repair including staging, timing, ventilation tube placement, types of hard and soft palate repair, and postoperative care (feeding-related elements and use of tongue suture, nasopharyngeal tube, arm restraints, and antibiotics).
