Samenvatting

Integratieve Gehechtheidsbevorderende Traumabehandeling voor Kinderen (IGT-K) richt zich op posttraumatische stresssymptomen, gehechtheidsproblemen, gedragsproblemen en problemen in de emotieregulatie bij chronisch getraumatiseerde kinderen. Dit hoofdstuk gaat over de gevolgen van ingrijpende gebeurtenissen voor het leven van een jeugdige en de specifieke problemen waar een problematische gehechtheid of een gehechtheidsstoornis toe kunnen leiden. Meervoudige interpersoonlijke traumatisering komt op jonge leeftijd veelvuldig voor, kan het neurobiologische systeem van het kind blijvend beïnvloeden en een breed spectrum aan klachten tot gevolg hebben. In de klinische praktijk blijkt dat deze kinderen niet enkel traumatische herinneringen moeten verwerken, maar dat bij hen ook herstel van communicatie en vertrouwen binnen het gezin nodig is. Daarnaast is bekend dat er een groep kinderen en gezinnen na kindermishandeling is waarbij bewezen effectieve behandelingen niet lukken, omdat de kinderen vanwege het brede klachtenpatroon hier nog niet ontvankelijk voor zijn. Voor deze groep kinderen is er nu een behandeling op maat: IGT-K. De rationale achter IGT-K wordt in dit hoofdstuk uiteengezet.

    De term ‘jeugdige’ is een breed begrip, namelijk iedereen jonger dan 23 jaar. Met kinderen wordt de leeftijdscategorie 0 tot 12 jaar bedoeld, met adolescenten die van 12 jaar tot de jongvolwassen leeftijd. In dit boek worden de termen door elkaar gebruikt en wordt vaak van kinderen gesproken ook wanneer deze ouder dan 12 jaar oud zijn. Het woord kind past dan beter, omdat het duidt op kind van de ouder.

    Omdat het niet mogelijk is deze term op elke plaats te vervangen voor een wetenschappelijk juiste benaming en omdat de praktische bruikbaarheid van dit boek voorop staat, is er voor gekozen de term gehechtheidstrauma over te nemen.

    Wanneer wij spreken van ouders, bedoelen wij ook gezinshuisouders, pleegouders, adoptieouders, tantes en ooms, grootouders en andere opvoeders.

    Onder negatieve cognities worden in dit boek zowel negatieve gedachten, leefregels als kernovertuigingen verstaan en niet altijd de negatieve cognities zoals die horen bij een target zoals wordt gebruikt in EMDR.

    Wanneer wij vanwege de leesbaarheid spreken van hij/hem/zijn kan ook zij/haar/haar gelezen worden en andersom.

  6. 6.

    Het gaat hierbij om een therapeut die ervaring heeft met systemische interventies en betrekken van het bredere systeem. Dit kan een systeemtherapeut zijn van de Nederlandse Vereniging voor Relatie- en Gezinstherapie (NVRG), maar ook een gezondheidszorg (GZ)-psycholoog, klinisch psycholoog of een kinderpsychiater die wordt ingezet als gezinstherapeut. Voor de leesbaarheid is gekozen voor de term gezinstherapeut, omdat niet iedere therapeut met ervaring in systemisch werken een NVRG-geregistreerd systeemtherapeut is.
