Zusammenfassung
Hinter dem Begriff Thrombotische Mikroangiopathien (TMA) verbergen sich unterschiedliche Erkrankungen, bei denen es zu einer Schädigung des Endothels mit konsekutivem thrombotischem Verschluss von kleinen Gefäßen in verschiedenen Organen kommt. Typisch ist für diese Erkrankungen folgende Trias: Thrombopenie, hämolytische Anämie mit Fragmentozytenbildung und Organschädigung (insbesondere Niere und Gehirn). In der Schwangerschaft bzw. frühen postpartalen Zeit werden zwei TMA-Erkrankungen gehäuft beobachtet: 1) Das atypische hämolytisch urämisches Syndrom (aHUS), welches durch eine Störung in der Komplementregulation ausgelöst wird. 2) Die Thrombotische thrombozyopenische Purpura (TTP), welche durch eine verminderte ADAMTS13-Aktivität auf dem Endothel zur Freisetzung sehr großer, thrombogen wirkender von-Willebrand-Faktor-Multimere im Blut führt. Diese beiden Erkrankungen sind unter anderem vom HELLP-Syndrom, Sepsis mit und ohne disseminierte Gerinnungsstörung, Vitamin-B12- oder Vitamin-B9-Defizienz, systemischem Lupus erythematodes und Antiphopholipidsyndrom abzugrenzen. Da für das aHUS und die TTP in den letzten 10 Jahren neue Medikamente auf dem Markt gekommen sind, ist diese Differenzierung auch therapeutisch von Bedeutung.
Bei Verdacht auf TMA sollte immer ein Internist (vorzugsweise Nephrologe oder Hämatologe) hinzugezogen werden.
