Zusammenfassung

Jede außerhalb des Cavum uteri lokalisierte Schwangerschaft ist eine Extrauteringravidität (EUG), die in >90 % der Fälle im Bereich der Tube stattfindet (Tubargravidität). Die Inzidenz liegt bei 1–2 %.

Das klinische Bild erstreckt sich von der völlig asymptomatischen Patientin über Unterbauchschmerzen mit vaginalen Schmierblutungen und einem akuten Abdomen bis hin zum hämorrhagischen Schock nach Tubarruptur. Die Vaginalsonografie bildet die Grundlage der Diagnostik und wird bei nicht gesichertem Befund durch den quantitativen Serum β-hCG-Verlauf ergänzt.

Neben der operativen Therapie – laparoskopische Sal**otomie und Sal**ektomie – haben sich konservative Therapiealternativen wie die systemische Gabe von Methotrexat etabliert. Ein rein abwartendes Vorgehen, mit einer eventuellen Spontanresorption als Ziel, kann bei fallendem Serum ß-hCG als Therapieoption in Erwägung gezogen werden und zeigt eine Erfolgsquote von bis zu 30 %.
