Zusammenfassung
Häufigste Ursache eines thorakalen Aortenaneurysmas ist die Atherosklerose, seltene Ursachen sind die zystische Medianekrose und Traumen. Pathologisch-anatomisch können 2 Formen unterschieden werden: das chronische Aortenaneurysma, hervorgerufen durch eine Dilatation der Aortenwand, und die akute Dissektion der Aorta, also das dissezierende thorakale Aortenaneurysma oder akute Aortensyndrom, hervorgerufen durch einen Riss der Intima, über den Blut zwischen die innere und äußere Schicht der Lamina media der Aortenwand eindringt und ein falsches Lumen bildet. Die Operation erfolgt mit der Herz-Lungen-Maschine. Wegen des hohen Operationsrisikos und des komplexen chirurgischen Vorgehens ist die Anästhesie bei diesen Eingriffen eine besondere Herausforderung für den Anästhesisten. Blutungen, kardiovaskuläre Störungen, Myokardinfarkt, respiratorische Insuffizienz, Nierenversagen, Schlaganfall und Rückenmarkschädigung gehören zu den wichtigsten Komplikationen dieser Eingriffe.
