Zusammenfassung
Bei 32 hochmyopen Augen von 27 Patienten mit einem Durchschnittsalter von 61 (40–86) Jahren bestand eine signifikante Katarakt. Außergewöhnlich waren die echographisch ermittelten Bulbuslängen, die im Mittel 33,54 mm betrugen (31,21 mm bis über 35,0 mm). Bei allen Patienten konnte eine Kataraktoperation komplikationslos durchgeführt werden. Es wurden einstückige, konvex/konkave PMMA-Linsen mit 6,5 mm Optik- und 13,5 mm Gesamtdurchmesser implantiert. Die Brechkräfte der implantierten Kunstlinsen lagen zwischen -1,0 und -8,0 dpt. Der Nachbeobachtungszeitraum betrug im Mittel 34 Monate (16–60 Monate).
Bei 30 der 32 Augen (94%) konnte eine Visusverbesserung festgestellt werden. Bei 2 Augen entsprach der postoperative dem präoperativen Visus. Die postoperative Refraktion lag bei 72% der Augen innerhalb ± ldpt von der Zielrefraktion.
Auch bei einem Nachbeobachtungszeitraum von bis zu 5 Jahren konnten keine operationsbedingten Netzhautkomplikationen, insbesondere zystoide Makulaödeme oder rhegmatogene Netzhautablösungen festgestellt werden. Die Gründe für das Ausbleiben von derartigen Komplikationen könnten in der Glaskörperstruktur und in der Operationsmethode liegen.
Die Phakoemulsifikation mit geeigneter Technik stellt bei hochmyopen Patienten kein erhöhtes Risiko dar. Die Patienten profitieren von der Minuslinsenimplantation bezüglich der postoperativen Refraktion. Darüber hinaus dient das Implantat der intraokularen Stabilisierung. Nach Kataraktoperationen bei hochmyopen Patienten sollte auf die Implantation von Minuslinsen nicht verzichtet werden.
Summary
In 32 eyes of 27 highly myopic patients a cataract extraction with implantation of a negative-power intraocular lens (IOL) was performed. All patients showed a significant cataract. The mean axial length of the eyes, measured by ultrasound, was 33,54 mm (31,21 up to more than 35 mm). We performed one ECCE. All other patients were operated on by phacoemulsification, usually with a self-sealing frown incision, but twice in combination with trabeculectomy. The IOL power varied from -1,0 to -8,0 diopter. We chose a one-piece, convex/concave PMMA model with a 6,5 mm optic and 13,5 mm diameter. There were no intraoperative complications. We reached a follow-up of 34 months (16–60 months). In 94% of the patients (30 of 32) we achieved an improvement of best corrected visual acuity. The same pre- and postoperative visual acuity were found in two eyes.
A postoperative refraction of 72% of the operated eyes were found within 1 diopter from the predicted power. Even after a follow-up of 1–5 years, there were no retinal complications, e.g., retinal detachment or CME.
In summary we believe that cataract extraction can be performed in highly myopic eyes if necessary. For reason of postoperative refraction and intraocular stability, an IOL should be implanted, even if negative lens power is required.
