Zusammenfassung

Wir untersuchten retrospektiv die postoperativen Ergebnisse nach Kataraktextraktion bei Patienten mit einer Bulbuslänge über 27 mm, die im Zeitraum von 1990 bis 1993 an unserer Klinik operiert wurden. Es konnten 88 Patienten dokumentiert werden, bei denen 131 Augen operiert wurden. Die mittlere Nachbeobachtungszeit betrug 18 Monate. 48 Augen (37,4%) erreichten einen Visus von mehr als 0,5; der Median des Visus stieg von 0,1 präoperativ auf 0,4. An Komplikationen traten intraoperativ eine Ruptur der hinteren Kapsel bei 5 Augen (3,8%), postoperativ ein Tensioanstieg bei 6 Augen (4,6%), eine Cataracta secundaria 51mal (39%), ein zystoides Makulaödem 2mal (1,5%) und eine Ablatio retinae bei 2 Patienten (1,5%) auf. Aufgrund der Korrektur des refraktiven Fehlers und dem relativ geringen Risiko in unserer Serie befürworten wir bei hochmyopen Patienten eine frühzeitige Kataraktextraktion.

Summary

We investigated the postoperative outcome of highly myopic patients (length of bulbus more than 27 mm) who underwent cataract extraction in our clinic between 1990 and 1993. We documented 88 patients (131 eyes). The mean follow-up time was 18 months. In 48 eyes (37.4%) a visual acuity of more than 0,5 was achieved, while the median rose from 0,1 before operation to 0,4. The following complications were observed: rupture of the posterior capsula in five eyes (3,8%), high eye pressure in six eyes (4,6%), cataract secundaria in 51 (39%), cystoid macular edema in 2 (1,5%) and retinal detachment in two patients. Because of the correction of the refractive errors the relatively low risk in our patients, we recommend early cataract extraction even in highly myopic patients.
