Zusammenfassung
Die Ursachen der Pouchitis nach ileoanaler Pouchanlage wegen Colitis ulcerosa (C. U.) sind nach wie vor unbekannt Ischämie des Pouches und die unphysiologische Stase des Stuhles im Ileumreservoir, aus der ein Überwucherung Gram-negativer Bakterien resultiert, werden für die Entstehung der Pouchitis verantwortlich gemacht. Daneben wird ein der C. U. ähnlicher Pathomechanismus postuliert. Dies drückt sich in einem ähnlichen Cytokinverhalten [1] wie die Colitis ulcerosa aus. Ebenso bleibt die Pouchitis bei ileoanalen Pouchanlagen anderer Indikationen (FAP, multilokuläres Colon-Ca) nahezu vollkommen aus.
