Summary
A major problem in the treatment of acute liver failure is a lack of therapeutic options for increasing intracerebral pressure (ICP). This animal model shows the efficacy of a bioartificial liver support system (BR) to treat an elevated ICP. Fifteen pigs were monitored for ICP, and cardiopulmonary and laboratory functions after hepatectomy and portocaval shunting. In seven pigs, with additional treatment of BR (primary porcine hepatocytes), stable ICP and a benefit in survival were measured. Untreated pigs (n = 8) showed continuous increasing ICP up to 35 mm Hg.
Zusammenfassung
Ein klinisches Problem bei akutem Leberversagen ist die mangelnde Therapiemöglichkeit des erhöhten intracerebralen Drucks (ICP). Tierexperimentell kann durch den Einsatz eines biohybriden Leberersatzverfahrens (BR) ein stabiler ICP erreicht werden. 15 Hausschweine wurden nach totaler Hepatektomie und Anlegen eines portocavalen Shuntes überwacht. Beobachtungsparameter waren ICP kardiopulmonale sowie laborchemische Parameter. In der Gruppe (n = 7) mit Behandlung (BR mit primären porcinen Hepatozytenkultur) wurde ein konstanter, nicht erhöhter ICP bei verlängertem Überleben gemessen. In der Gruppe ohne BR-Behandlung (n = 8) stieg der ICP kontinuierlich auf Werte bis zu 35 mmHg.
