Abstract
Medical practice is described as shrouded in myths, one of which being the unexplained strategies of decision making in medical practice. After a brief review of the existing theories and models of medical decision making, a definition is made and the components enumerated. The process of diagnostic decision making is modeled, based on the assumption that the inference mechanism is a trained cognitive skill. It develops as the knowledge base is built and the methodology of clinical examination is learnt. The issues in the “informatization” of the model are briefly discussed.
Zusammenfassung
Medizinisches Handeln wird beschrieben als mystisches Geheimnis, so auch die ungeklärten Strategien des ärztlichen Entscheidungsprozesses. Nach einem kurzen Rückblick auf bestehende Theorien und Modelle zur ärztlichen Entscheidungsfindung wird eine Definition gegeben und die Komponenten des diagnostischen Prozesses werden benannt. Ausgehend von der Annahme, daB der menschliche ‘Inferenz-Mechanismus’ eine gelernte kognitive Fähigkeit ist, wird ein Phasenmodell über den Prozess der Diagnostik (von der Hypothesenbildung zur Diagnosenstellung) beschrieben. Diese Fähigkeit entwickelt sich mit dem Aufbau der menschlichen ’Wissensbasis’ und dem Erlernen der Methoden der klinischen Untersuchung. Probleme der Umsetzung dieses Modells mit Methoden der Informatik werden kurz angerissen.
