Summary

Radical non-anatomical resection of liver metastases was performed in 21 patients using the ultrasound surgical aspirator. There were two recurrences in different segments of the liver within the followup period of 15 months. None of the patients died. By using the ultrasound surgical aspirator, perioperative blood transfusions were reduced by 30% and the postoperative complication rate dropped from 20% to 5%. Operation time did not increase significantly. Atypical resection is sufficient in 50% of the patients for radical removal of metastases. Resection of the liver using the ultrasound surgical aspirator is low-risk and parenchyma-saving technique.

Zusammenfassung

Bei 21 Patienten wurde mit Hilfe des Ultraschallaspirators eine radikale Metastasenextirpation atypisch durchgeführt. Im Beobachtungszeitraum von 15 Monaten traten 2 Rezidivmetastasen in jeweils anderen Lebersegmenten auf, kein Patient verstarb. Im Vergleich zur konventionellen Resektionstechnik ließ sich der perioperative Transfusionsbedarf um 30% und die p. op. Komplikationsrate von 20 auf 5% senken. Die Operationszeit ist nicht wesentlich verlängert. Das atypische Vorgehen ist in ca. 50% der Fälle zur radikalen Metastasenextirpation ausreichend. Die Ultraschallaspiration ist ein komplikationsarmes und parenchymsparendes Resektionsverfahren.
