Abstract

Anterior lumbar exposure has been used as an approach for spinal fusion. While very effective for managing appropriately selected spinal disease, complications may ensue. The retroperitoneal nature of the exposure and proximity to the vasculature leads to risk of vascular injury. Patients with higher risk potential can be identified with a thorough medical history and review of previous lower abdominal surgery. The most common vascular injury is a venous injury, and early identification and management is key to a successful outcome. This chapter discusses comorbidities associated with increased perioperative risk, the most common categories of injuries and technical considerations for hemostatic management.
