Abstract

Acute pancreatitis (AP) represents a common potentially life-threatening disease mostly related to gallstone or alcohol abuse with an overall mortality rate of around 5%. Despite the reported incidence burden on 30 to 40-year-old persons, a higher peak is also reported in persons older than 65 years who usually fall an easy prey to this disease due to compromised body systems. For an adequate management, a central role is represented by a mix of prompt diagnosis, by a precise radiological/computed tomography (CT) scan-mediated classification (Atlanta Classification and Determinant-based classification), and by the clinical severity scores (e.g., Ranson criteria, Acute Physiology and Chronic Health Evaluation II (APACHE II), Simplified Acute Physiology Score (SAPS II), Bedside Index of Severity in Acute Pancreatitis (BISAP), etc.). This chapter explores the impact of age and personal fragility (comorbidities and chronically ill) on disease severity and subsequent medical/surgical options.
