Abstract

Vascular anomalies comprise a spectrum of diseases that can present with both bleeding and thrombotic risk. Pediatric hematologists and oncologists are often consulted for the diagnosis and management of coagulopathies associated with these conditions. In this chapter, we review the pathophysiology, epidemiology, and treatment of localized intravascular coagulopathy (LIC) associated with venous malformations as well as disseminated intravascular coagulation (DIC) seen with Kasabach-Merritt phenomenon and vascular tumors. We discuss periprocedural bleeding and thrombotic risk assessment and management and review characteristics of coagulopathies that distinguish the different vascular anomalies. Lastly, we discuss treatment options with anticoagulants, as well as growing experience with the newer direct oral anticoagulants (DOACs) and mTOR inhibitors.
