Résumé
Actuellement environ 10 à 15 % des accouchements par voie basse nécessitent une aide instrumentale, ventouse, forceps ou plus rarement spatules. Des complications chez le nouveau-né peuvent être observées, notamment au niveau de l’extrémité céphalique. Elles sont rares et le plus souvent bénignes. Néanmoins, lorsque l’extraction fœtale a été difficile, des complications plus graves peuvent survenir, comme l’hématome extensif du cuir chevelu, certaines hémorragies intracrâniennes ou des fractures du crâne. Elles dépendent pour une part de l’instrument utilisé mais aussi du contexte obstétrical et notamment du rôle délétère d’une asphyxie per-partum. Cette dernière expose le nouveau-né au risque d’encéphalopathie anoxo-ischémique. Dans ces circonstances, le pédiatre devra être à même de prendre en charge dès la naissance ce nouveau-né, de dépister et de traiter des complications qui pourraient apparaître secondairement. Une information claire devra être donnée aux parents quant aux différents mécanismes lésionnels.
