Abstract
Sur les quelque 247 millions de cas de paludisme dénombrés chaque année dans le monde, 86 % (212 millions de cas) sont déclarés en Afrique et tuent 800 000 personnes (surtout des enfants). La plupart des décès touchent les plus démunis qui n’ont pas accès à des mesures de prévention et à des traitements adéquats (Rowe et al. 2006). Comme ailleurs en Afrique subsaharienne, le paludisme est le principal problème de santé publique en Afrique de l’Est (Burundi, Kenya, Rwanda, Tanzanie et Ouganda). Soixante-treize pour cent des 115 millions d’habitants de la région sont exposés au risque d’infection palustre. Le paludisme est responsable de plus d’un tiers des décès des enfants de moins de cinq ans et d’un cinquième des décès de femmes enceintes (OMS 2008). La situation est très différente parmi les pays voisins, et même dans des régions géographiques bien circonscrites, pour ce qui est de l’infectiosité des moustiques vecteurs, de l’intensité des contacts humains-vecteurs et de la prévalence de la maladie. En Afrique de l’Est, la charge de morbidité et la persistance du paludisme dépendent de facteurs anthropogènes, topographiques et écologiques.
