Zusammenfassung
Die Induktionschemotherapie mit TPF (Docetaxel, Cisplatin, 5-Fluorouracil) scheint bei Plattenepithelkarzinomen des Kopf-Hals-Bereichs derzeit weltweit eine Renaissance zu erfahren. Dem ungebrochen hohen Stellenwert der Chirurgie mit ergänzender Bestrahlung bzw. Radiochemotherapie steht bislang die primäre simultane platinbasierte Radiochemotherapie bei nicht resezierbaren Tumoren gegenüber. Aufgrund der ermutigenden Erfahrungen der Chemoselektion im Rahmen einer Induktionschemotherapie ist diese v. a. bei nur unter Organverlust resezierbaren Tumoren wieder in den Fokus des Interesses klinischer Studien gerückt. Die hohen Ansprechraten, aber auch die toxizitätsbedingte Mortalität verdeutlichen, dass eine sorgfältige Indikationsstellung, aber auch Modifikation der Dosierung und Hinzuziehung neuer Substanzen derzeit Gegenstand klinischer Studien sein müssen. Außerhalb von Studien kann die TPF-Anwendung momentan nicht empfohlen werden.
Abstract
Induction chemotherapy using TPF (docetaxel, cisplatin, 5-fluorouracil) in squamous cell carcinomas of the head and neck area currently appears to be experiencing a worldwide renaissance. The persistently high value of primary surgery with adjuvant radiation or chemoradiation stood in opposition to primary radiochemotherapy in non-resectable disease until today. Due to the encouraging results seen with chemoselection in the context of induction chemotherapy, the latter, particularly in the case of tumors only resectable with organ loss, has come back into the focus of clinical trials. In spite of encouragingly high response rates, TPF also showed increased mortality rates following TPF-induced neutropenic sepsis, highlighting the need for careful indication. In addition, on-going studies are focusing on dose modifications and additional combinations with EGFR-targeting. Thus induction chemotherapy in head and neck cancer is currently not recommended outside clinical trials.
