Zusammenfassung
Hintergrund
Die Altersbestimmung zur Personenidentifikation hat in der Rechtsmedizin grundlegende Bedeutung. Eine Vielzahl an Methoden ist hierzu in der Literatur beschrieben. Genauigkeit und Zuverlässigkeit dieser Methoden sind jedoch eingeschränkt. Die Reevaluierung bestehender und die Entwicklung neuer Methoden sind daher stetig notwendig.
Ziel der Arbeit
Neue nummerische Methoden der automatischen Bildanalyse im Rahmen der Alters- und Geschlechtsbestimmung, die im forensischen oder anthropologischen Kontext relevant sind, werden evaluiert.
Material und Methode
Schädel europäischer Herkunft im Altersbereich von 20 bis 101 Jahren wurden mit einem hochauflösenden Flat-Panel-Computertomographen (Flat-Panel-CT) radiologisch untersucht. Datensätze von 632 Kalottenhälften (223 weiblich, 409 männlich) wurden erhalten, bestehend aus insgesamt 214.880 Schnittbildern. Im Rahmen des Projekts wurde eine Software entwickelt, die Liniensegmente erzeugt, die die jeweilige Kalotte schneiden. Kalottenkanten werden anhand der Hounsfield-Einheiten (HE; mit der Knochendichte korrelierende Grauwerte in einem CT-Bild) detektiert. Die Eigenschaften der Liniensegmente werden zur Untersuchung der „Kalottendicken“, der „Standardabweichungen der Kalottendicke“, der „mittleren HE entlang der Schnittlinien durch die Kalotten“ und der „mittleren HE-Standardabweichung entlang der Schnittlinie“ herangezogen sowie geschlechtsspezifisch mit dem Alter korreliert. Hauptinteressen dieser Studie sind die Untersuchung der „Standardabweichungen der Kalottendicke“ und der „mittleren HE-Standardabweichung entlang von Schnittlinien“, da diese bislang noch nicht erforscht wurden. Insgesamt bildeten ca. 3.900.000 untersuchte Liniensegmente die Grundlage dieser Studie.
Ergebnisse
Weder die Kalottendicken noch die Standardabweichungen der Kalottendicken korrelierten mit dem Alter. Die Knochendichte nahm bei den Frauen mit zunehmendem Alter leicht ab (Korrelationskoeffizient r der Kalottenregion: frontal − 0,4; okzipital − 0,5); bei den Männern fanden sich r-Werte nahe 0. Die Standardabweichungen der HE entlang der Kalottenschnittlinien nahmen bei den Frauen mit zunehmendem Alter geringfügig ab (r: frontal − 0,5; okzipital − 0,6). Bei den Kalotten männlichen Geschlechts zeigten sich r-Werte nahe 0.
Schlussfolgerung
Kalottendicken korrelieren nicht mit dem Alter. Die Standardabweichung der Kalottendicken, die als Maß für die Rauheit oder die geometrische Unregelmäßigkeit interpretiert wird, korreliert ebenfalls nicht mit dem Alter. Die Abnahme der Knochendichte mit dem Alter bei Frauen, im Gegensatz zu Männern, ist literaturbekannt und konnte in der vorgestellten Studie bestätigt werden. Die Abnahme der Standardabweichung der HE entlang der Kalottenschnittlinien mit dem Alter zeigt, dass die geometrische Knochendichtevariabilität weiblicher Schädelkalotten mit zunehmendem Alter geringfügig abnimmt: Dies ist anders als bei Männern; hier bleibt dieser Parameter konstant. Durch sehr stark streuende Werte sind auch die bei Frauen gefunden Korrelationen allerdings zu gering, um eine Altersbestimmung von praktischer Relevanz zu ermöglichen.
Abstract
Background
Age determination for personal identification is of fundamental importance in forensic medicine and many methods have been described in the literature. However, the accuracy and reliability of these methods are limited; therefore, a reevaluation of existing methods and the development of new methods is continually necessary.
Aim
New numerical and automated image analysis methods were evaluated for age and sex determination, which are of relevance in a forensic or anthropological context.
Material and methods
Skulls of European origin from individuals aged 20–101 years at death were examined with a high-resolution flat-panel computed tomography (flat-panel CT) system. Datasets comprising 214,880 series images were acquired from 632 skull halves (223 female and 409 male). Software was developed that generates line segments that intersect the calvarium. The margins of these line segments were detected by using Hounsfield units (HU), which are gray level values that correlate with bone density in CT images. The properties of these line segments were investigated and correlated with age and sex, with respect to skull thickness, the standard deviation of the skull thickness, the mean HU along the line segment through the skull, and the mean HU standard deviation along the line segment intersecting the skull. The main interests of this study were the analysis of the standard deviation of the skull thickness and the mean HU standard deviation along the line segment intersecting the skull as these criteria have not yet been investigated. This study is based on the examination of approximately 3,900,000 line segments.
Results
Neither skull thickness nor the standard deviations for skull thickness correlated with age. For females, bone density decreased slightly with increasing age: the correlation coefficient (r) for the frontal cranial region was − 0.4 and − 0.5 for the occipital region and for males the r-values were close to 0. Standard deviations for the HU values along the calvarial intersection lines decreased slightly for females with increasing age (frontal region r = − 0.5, occipital region r = − 0.6) and for males the r-values were close to 0.
Conclusion
As known, skull thickness and age are not correlated. The standard deviation for skull thickness, which is interpreted as a measure of the roughness or geometrical irregularity of bone also did not correlate with age. The observation documented in the literature that in contrast to males bone density in females decreases with increasing age was further confirmed by these findings. The decrease in the standard deviations for the HU values along the calvarial intersection lines with increasing age of females indicated a slight decrease in the variability in geometric bone density in female skulls with age. In male skulls this parameter remained constant. Because the data showed a broad scattering even the slight correlations found for females did not enable an aging method of practical relevancy.
