Zusammenfassung
Eine Revisionsplastik des vorderen Kreuzbands (VKB) kann in der Regel erst nach einer Auffüllung der erweiterten Bohrkanäle mit Spongiosa oder Allograft erfolgen. Dieser zusätzliche Eingriff führt zu operativen Risiken und einer Verlängerung der Behandlungsdauer. Das vorübergehende Fehlen des Transplantats kann zudem durch die damit einhergehende Instabilität zur weiteren Schädigung des Kniegelenks führen. In diesem Beitrag soll eine modifizierte Knochen-Sehnen-Knochen-Technik („bone-tendon-bone“, BTB) vorgestellt werden, bei der durch eine geänderte Anlage des tibialen Bohrkanals und der Möglichkeit einer zusätzlichen Spongiosaentnahme aus dem Tibiakopf eine einzeitige Revision ohne zusätzliche Fixation möglich ist. Im 1‑Jahres-Follow-up erwies sich die Technik bereits als effizientes Verfahren mit gleichwertigen Ergebnissen zu den bisher üblichen Verfahren bei deutlich verkürzter Gesamtbehandlungsdauer.
Abstract
Anterior cruciate ligament (ACL) revision surgery can usually only be performed after filling of the bone defect caused by fixation screws with cancellous bone or an allograft. This additional procedure leads to surgical risks and an extension of overall duration of treatment. Moreover, the temporary absence of an ACL transplant can lead to further damage to the affected joint due to the associated instability. This article presents a modified bone-tendon-bone (BTB) procedure, which enables single-stage revision treatment without the use of additional fixation material, due to an altered placement of the tibial drill canal with the option of additional cancellous bone harvesting from the tibial head. In a 1‑year follow-up the technique proved to be an efficient procedure with comparable results to the currently common procedures and with a significantly reduced overall duration of treatment.
