Zusammenfassung
Etwa 9 % aller hinteren Kreuzbandverletzungen (HKB) sind knöcherne Ausrisse meist am tibialen Ansatz. Neben einer Röntgenaufnahme in der Primärphase empfehlen wir grundsätzlich bei Verdacht auf einen Kniebinnenschaden eine Kernspintomographie, bei V. a. knöcherne Verletzungen zusätzlich ein CT. Knöcherne HKB-Ausrisse entstehen meist durch Hochrasanztraumata, zusätzliche begleitende Extremitätenverletzungen sind häufig und müssen sowohl in der Diagnostik als auch beim Therapiealgorithmus berücksichtigt werden. Die konservative Therapie wird nur bei nichtdislozierten Frakturen empfohlen. Auch bei nur minimaler Dislokation raten wir zur operativen Versorgung. Bei relevanten intraartikulären Begleitpathologien werden die Arthroskopie des Kniegelenks und die entsprechende Adressierung der Begleitverletzungen durchgeführt: in diesem Fall auch arthroskopische Versorgung des knöchernen HKB-Ausrisses mit hoch reißfesten Polyethylenfäden. Bei fehlenden intraartikulären Begleitpathologien wird der knöcherne HKB-Ausriss über einen minimalinvasiven Zugang in Bauchlage ohne Arthroskopie versorgt. Die dadurch mögliche direkte Verschraubung ist technisch einfach, bietet eine hohe Primärstabilität und damit eine frühfunktionelle und schnellere Rehabilitation. Die klinischen Ergebnisse sind auch im Langzeitverlauf in der Regel gut und exzellent. Die immer wieder beobachtete Ausbildung einer Pseudarthrose im Rahmen der konservativen Therapie kommt nach operativer Stabilisierung kaum vor.
Abstract
Approximately 9% of acute posterior cruciate ligament (PCL) injuries are bony avulsions, mostly located at the tibial insertion site. In addition to a plain x‑ray image in the primary stage we recommend an MRI or a CT scan if a bony injury is suspected. Bony avulsions of the PCL mostly result from high-velocity trauma. Concomitant injuries of the lower extremities are commonly found and should be considered in the diagnostic and therapeutic algorithm. Conservative treatment is recommended only in nondisplaced fractures, while minimally displaced fractures should be surgically treated. Relevant intraarticular concomitant injuries should be arthroscopically addressed. In addition, bony avulsions of the PCL can be reduced and fixed with high tear-resistant polyethylene threads. In the absence of intraarticular concomitant injuries a minimally invasive dorsal approach with the patient in prone position can be used to directly reduce and fix the bony avulsion, using screw fixation. This technique is technically simple, provides a high primary stability and therefore an early functional and quick rehabilitation. Clinical long-term results are good and excellent. Nonunions, as often seen after conservative treatment, are barely observed.
