Zusammenfassung
Symptomatisch wirksame Therapien für neurodegenerative Erkrankungen sind auf Grundlage des Wissens über pathophysiologische und neurochemische Zusammenhänge im zentralen Nervensystem in den letzten 50 Jahren entwickelt worden. Sie stellen bisher die Basis der verfügbaren Therapieoptionen dar. Die Kenntnis pathophysiologischer und neurochemischer Zusammenhänge ist nicht nur für die Entwicklung von Therapien notwendig, sondern ermöglicht auch einen sinnvollen Einsatz der zur Verfügung stehenden Substanzen. Anhand der Parkinson-Krankheit und der Demenz vom Alzheimer-Typ sollen kurz die wichtigsten neuropathologischen und neurochemischen Veränderungen dargestellt werden, die diese Erkrankungen charakterisieren. In den letzten Jahren hat sich die Entwicklung neuer Substanzen von den symptomatischen hin zu den krankheitsmodifizierenden Therapieoptionen entwickelt; letztere fußen meist auf den neuropathologisch nachweisbaren Veränderungen. Neuere Ergebnisse aus klinischen Studien lassen hoffen, dass in absehbarer Zeit krankheitsmodifizierende Therapieoptionen für neurodegenerative Erkrankungen zur Verfügung stehen werden.
Abstract
Symptomatically effective forms of treatment for neurodegenerative diseases have been developed in the last 50 years based on the knowledge about the pathophysiological and neurochemical context in the central nervous system. These so far represent the basis of available treatment options. Knowledge of the pathophysiological and neurochemical context, however, is not only necessary for the development of treatment but also enables a meaningful implementation of currently available substances. The most important neuropathological and neurochemical alterations that characterize Parkinsonʼs disease and Alzheimer type dementia are briefly presented. In recent years, new substances ranging from symptomatic to disease-modifying treatment options have been developed, the latter mostly based on the neuropathologically detectable alterations. Recent results from clinical studies raise hopes that disease-modifying treatment options for neurodegenerative diseases will become available in the foreseeable future.
