Zusammenfassung
Die neuen Leitlinien der European Society of Cardiology (ESC) zur Herzinsuffizienz von 2021 nehmen eine subtile, klinisch aber relevante Neubewertung der Kategorisierung der Herzinsuffizienz anhand der linksventrikulären Ejektionsfraktion (LVEF) vor. Neben der Herzinsuffizienz mit reduzierter Pumpfunktion (HFrEF; LVEF ≤ 40 %) spricht sie von Herzinsuffizienz mit mäßiggradig reduzierter (HFmrEF; LVEF 41–49 %) sowie von Herzinsuffizienz mit erhaltener Pumpfunktion (HFpEF; LVEF ≥ 50 %). Dies impliziert, dass für Patienten mit HFmrEF auf der Basis gepoolter Daten und retrospektiver Analysen nun eine Therapie in Analogie zur HFrEF empfohlen wird. Die Diagnose HFpEF beruht auf klinischen Zeichen, einer objektivierten strukturellen oder funktionellen kardialen Dysfunktion und zusätzlich erhöhten Werten des natriuretischen Peptids vom B-Typ sowie einem erhöhten enddiastolischen linksventrikulären Füllungsdruck. Bei nicht eindeutiger Konstellation soll zur Absicherung der Diagnose ein Belastungstest durchgeführt werden. In der Therapie der HFpEF steht weiterhin die Identifikation und gezielte Therapie von Komorbiditäten im Vordergrund. Diuretika werden bei Stauungszeichen zur symptomatischen Therapie empfohlen. Die Behandlung von Patienten mit Diabetes und Herzinsuffizienz soll einen Natrium-Glukose-Kotransporter-2(SGLT-2)-Inhibitor (Gliflozin) enthalten. Alle Patienten mit HFpEF sollen in einem multidisziplinär ausgerichteten Herzinsuffizienzmanagementprogramm versorgt werden, das Strategien für die Stärkung der Selbsthilfe („self-empowerment“) bereithält und die Möglichkeit zur Teilnahme an einem körperlichen Trainingsprogramm bietet. Vor Kurzem konnte erstmals gezeigt werden, dass sich harte klinische Endpunkte durch Gabe des SGLT-2-Inhibitors Empagliflozin reduzieren lassen. Die Berücksichtigung dieser Ergebnisse in aktualisierten Empfehlungen darf zeitnah erwartet werden.
Abstract
In August 2021, an update of the European Society of Cardiology—Heart Failure Association guideline for the diagnosis and treatment of heart failure was released. To review the changes implied by current guidelines regarding the diagnosis and treatment of patients with heart failure and preserved left ventricular ejection fraction (HFpEF). The diagnosis of HFpEF requires the combined presence of clinical signs, left ventricular ejection fraction ≥ 50%, elevated natriuretic peptides, and elevated left ventricular filling pressure. If the diagnosis remains equivocal, a stress test is recommended. The targeted identification and treatment of comorbid conditions is key for a holistic therapeutic approach to HFpEF. Diuretics are recommended in congested patients with HFpEF in order to alleviate signs and symptoms. The treatment of diabetic patients with heart failure should include a sodium glucose co-transporter‑2 (SGLT2) inhibitor. All patients with HFpEF should be enrolled in a multidisciplinary heart failure management program aiming to improve self-care strategies and offer participation in an exercise program. It was recently shown for the first time in a randomized trial that hard clinical endpoints could be reduced in patients with HFpEF using the SGLT2 inhibitor empagliflozin. It is expected that this finding will become part of updated treatment recommendations in the near future. Although challenging, the early diagnosis of HFpEF is key to averting the poor prognosis associated with this frequent condition. Multidisciplinary care and innovative pharmacologic and non-pharmacologic therapies, however, can improve quality of life, exercise tolerance, and prognosis.
