Zusammenfassung
Hintergrund
Das Rezidiv des Rektumkarzinoms ist eine komplexe Erkrankung, die interdisziplinäre Zusammenarbeit und ein individualisiertes Vorgehen erfordert. Durch Fortschritte in der Indikationsstellung, der präoperativen Bildgebung, der Operationstechnik und der intra- und postoperativen Betreuung konnten Verbesserungen im früh-postoperativen Outcome sowie im onkologischen und funktionellen Langzeitergebnis erzielt werden.
Ziel der Arbeit
Ziel dieser Arbeit war es, einen Überblick über die aktuelle chirurgische Therapie des Rektumkarzinomrezidivs einschließlich präoperativer Diagnostik, differenzierter Operationstaktik und postoperativen Ergebnissen zu geben.
Material und Methoden
Es wurde eine Literaturrecherche mit den Suchbegriffen „rectal cancer recurrence“, „pelvic exenteration“, „preoperative staging“, „plastic reconstruction“, „surgical treatment of recurrent rectal cancer“ und „results after surgical resection of recurrent rectal cancer“ durchgeführt.
Ergebnisse
In erfahrenen Zentren mit interdisziplinärer Expertise werden Mortalitätsraten von 2 % in der chirurgischen Therapie des Rektumkarzinomrezidivs erreicht, die Rate an schweren Komplikationen liegt allerdings selbst in Zentren bei mindestens 30 %. Der wichtigste Faktor für das Langzeitüberleben ist das Erreichen einer R0-Situation. Gelingt die R0-Resektion, liegt das 5‑Jahres-Gesamtüberleben bei immerhin 30 %.
Diskussion
Durch Fortschritte in der Operationstechnik und perioperativen Versorgung haben sich die Grenzen der technischen Resektabilität in den letzten Jahren verschoben. Die Indikation zur operativen Therapie muss im interdisziplinären Kontext und nach ausführlicher Aufklärung des Patienten über perioperative Komplikationen, Morbidität, onkologische und funktionelle Prognose getroffen werden.
Abstract
Background
Recurrent rectal cancer is a complex disease which requires an interdisciplinary cooperation and an individualized approach. Advances in the indications for surgery, in preoperative imaging, in surgical techniques and perioperative care have led to improvements in the early postoperative outcome and oncological and functional long-term results.
Objective
The aim of this study was to give a review of the current surgical treatment of recurrent rectal cancer with a focus on preoperative diagnostics, differentiated surgical techniques and postoperative, oncological and functional outcomes.
Material and methods
A literature search was carried out with the terms “rectal cancer recurrence”, “pelvic exenteration”, “preoperative staging”, “plastic reconstruction”, “surgical treatment of recurrent rectal cancer” and “results after surgical resection of recurrent rectal cancer”.
Results
Mortality rates of 2% in the surgical treatment of recurrent rectal cancer can nowadays be achieved in specialized centers with interdisciplinary expertise; however, the rate of severe complications is around 30% even in centers. The most important factor for long-term survival is achieving a R0 situation. If an R0 situation can be achieved, the 5‑year overall survival is around 30%.
Conclusion
The limits of technical resectability have been extended in recent years by advances in surgical techniques and perioperative care. The indications for surgical resection must be based on an interdisciplinary decision and patients have to be informed in detail regarding perioperative complications, morbidity and oncological and functional prognosis before giving consent.
