Zusammenfassung

Anhand der Literatur und eigener Untersuchungen wird über die Taxonomie von Yersinia enterocolitica und über die Klinik, Pathogenese, Epidemiologie, Therapie und Diagnose der von ihr beim Menschen ausgelösten Infektionen berichtet. Die bisher bestehende diagnostische Unsicherheit durch Fehlen einheitlicher bakteriologischer und serologischer Untersuchungsverfahren wird aufgezeigt. 46 durch Erreger-und/oder Antikörpernachweis ätiologisch gesicherte Infektionen beweisen, daß außer der Erregerisolierung dem O-Agglutininnachweis mit gekochten Antigenen bei Infektionen mit Stämmen der Gruppen I und V die größte diagnostische Bedeutung zukommt. Der Antikörper-bzw. Erregernachweis gelang in 7,5 bzw. 1,9% der untersuchten Serum- bzw. Stuhlproben. Einzelheiten sind den tabellarischen Zusammenstellungen und kasuistischen Beiträgen zu entnehmen.

Summary

The taxonomic position of Yersinia enterocolitica as well as the clinical, pathogenetic, epidemiologic, therapeutic and diagnostic aspects of human infections are discussed based on data from the literature and data from our own laboratory. Emphasis is placed upon difficulties with regard to the diagnosis of these infections as evidenced by the lack of reliable and standardized bacteriologic and serologic tests. On the basis of 46 human cases, which were diagnosed through isolation of the causative agent and/or through serologic methods, it is shown that, apart from the demonstration of the etiologic agent, O-antigen agglutination tests employing boiled antigens, are reliable with respect to the serodiagnosis of O-group I and V infections due to Y. enterocolitica. It was possible to arrive at a definitive diagnosis in 7.5 per cent of the examined serum samples and in 1.9 per cent of the submitted stool samples.
