Summary
The effect of Carbutamide (N1-sulfanilyl-N2-butylcarbamide) on isolated perfused livers of normal and of alloxan-diabetic ketotic rats was studied. Preparation of animals and techniques of operation and perfusion were the same as described previously [52]. The concentration of carbutamide in the perfusion plasma was 1.20 · 10−3 M and that of glucose was in all experiments 11.1 · 10−3 M. — The following significant effects of Carutamide on liver metabolism were demonstrated: — 1. The net output of inorganic phosphate by normal and diabetic livers was significantly delayed. — 2. The elevated net output of potassium by diabetic livers was normalized. — 3. The net output of α-amino-acids by normal and diabetic livers was significantly reduced by Carbutamide, and a net uptake of α-amino-acids could even be demonstrated. — 4. The rise in the medium concentrations of lactate and pyruvate was increased by Carbutamide in experiments with diabetic livers. This effect was statistically significant for pyruvate. — 5. Bile production by normal and diabetic livers increased under Carbutamide. This effect was significant for the first 2 hours of the experiments. — The net glucose balance was not significantly influenced by Carbutamide in experiments with either normal or diabetic livers. In addition, an effect on the changes of liver glycogen could not be found. — Carbutamide did not inhibit formation of ketone bodies in experiments with either normal or diabetic livers. The kinetics of uptake of non-esterified fatty acids (NEFA) did not show an influence of Carbutamide. The plasma level of Carbutamide remained constant throughout the 3 hours of perfusion. These results support the hypothesis that the therapeutic effect of the blood sugarlowering sulfonyl-ureas depends not only on the ß -cytotropic effect, but also on a direct effect on liver metabolism.
Résumé
On a étudié l'effet du Carbutamide (N1-sulfanilyl-N2-butylcarbamide) sur les foies isolés et perfusés des rats normaux et des rats rendus diabétiques cétosiques par l'alloxane. — La préparation des animaux ainsi que les techniques d'opération et de perfusion étaient les mêmes que celles décrites antérieurement [52]. La concentration de carbutamide dans le plasma de perfusion était de 1.20 · 10−3 M. La concentration en glucose du milieu de perfusion était de 11.1 · 10−3 M dans toutes les expériences. — On a pu démontrer les effets significatifs suivants du carbutamide sur le métabolisme du foie: -1. La quantité nette de phosphate inorganique sortant des foies normaux et diabétiques était significativement diminuée. — 2. La quantité nette et élevée de potassium sortant des foies diabétiques se normalisait. — 3. La quantité nette d'acides α-aminés sortant des foies normaux et diabétiques était significativement réduite par le carbutamide et on a pu démontrer une nette captation des acides α-aminés sous l'effet du carbutamide. — 4. L'augmentation des concentrations de lactate et de pyruvate du milieu était accrue par le carbutamide dans les expériences sur les foies diabétiques. Cet effet était statistiquement significatif pour le pyruvate. — 5. La production de bile des foies normaux et diabétiques augmentait sous l'effet du carbutamide. Cet effet était significatif pendant les 2 premières heures des expériences. — Le bilan net du glucose n'était pas influencé significativement par le carbutamide, ni dans les expériences sur les foies normaux, ni dans celles sur les foies diabétiques. On n'a pas trouvé non plus d'effet sur les changements du glycogène hépatique. — Le carbutamide n'inhibait pas la formation des corps cétoniques, ni dans les expériences sur les foies normaux, ni dans celles sur les foies diabétiques. La cinétique de la captation des acides gras non-estérifiés (NEFA) n'était pas influencée par le carbutamide. Le taux plasmatique de carbutamide demeurait constant pendant les 3 heures de perfusion. — Ces résultats appuient l'hypothèse selon laquelle l'effet thérapeutique des sulfonylurées hypoglycémiants ne dépend pas seulement de l'effet ß-cytotrope, mais aussi de l'effet direct sur le métabolisme du foie.
Zusammenfassung
An isolierten, perfundierten Lebern von normalen und alloxandiabetischen ketotischen Ratten wurde die Wirkung von Carbutamid (N1-Sulfanilyl-N2-butylcarbamid) untersucht. — Die Vorbereitung der Tiere sowie die Operations- und Perfusionstechnik entsprachen früheren Angaben [52]. Die Carbutamidkonzentration im Perfusionsplasma betrug 1.20 · 10−3 M. Die Glucosekonzentration im Medium war in allen Versuchen 11.1 · 10−3 M. Die folgenden signifikanten Carbutamid-Bffekte auf den Leberstoffwechsel ließen sich nachweisen: — 1. Die Nettoabgabe von anorganischem Phosphat (Pi) durch normale und diabetische Lebern wurde signifikant vermindert. — 2. Die erhöhte Nettoabgabe von Kalium durch diabetische Lebern wurde normalisiert. — 3. Die Nettoabgabe von α-Aminosäuren durch normale und diabetische Lebern war unter Carbutamid signifikant vermindert bzw. es wurde eine Nettoaufnahme von α-Amino-säuren nachweisbar. — 4. Der Anstieg der Mediumkonzentrationen von Lactat und Pyruvat wurde in Experimenten mit diabetischen Lebern durch Carbutamid gesteigert. Der Effekt ist für Pyruvat signifikant. — 5. Die Galleproduktion normaler und diabetischer Lebern ist unter Carbutamid erhöht. Der Unterschied ist während der ersten 2 Versuchsstunden significant. — Weder die Netto-Glucosebalance normaler noch diejenige diabetischer Lebern wurde durch Carbutamid meßbar beeinflußt. Ebensowenig war ein Effekt auf das Verhalten des Leberglykogengehaltes nachzuweisen. Eine Hemmung der Ketonkörperbildung war weder in Experimenten mit normalen, noch in solchen mit diabetischen Lebern festzustellen. Auch die Kinetik der Aufnahme unveresterter Fettsäuren (NEFA) blieb unbeeinflußt. Der Carbutamidplasmaspiegel blieb während der 3-stündigen Versuchsdauer unverändert. — Die Ergebnisse unterstützen die Hypothese, daß der therapeutische Effekt der blutzuckerwirksamen sulfonylharnstoff-Derivate nicht nur auf einemβ-cytotropen Effekt, sondern außerdem auf einer direkten Wirkung auf den Leberstoffwechsel beruht.
