Zusammenfassung
Die Echinokokkosen des Menschen sind parasitäre Infektionen mit meist primärem Leberbefall. Es existiert die weltweit verbreitete zystische Echinokokkose (Hundebandwurm) sowie die alveoläre Echinokokkose, ein tumoröses Krankheitsbild (Fuchsbandwurm), welches auch lokal in Süddeutschland und angrenzenden Regionen vorkommt. Beide Formen weisen langjährige Inkubationszeiten auf, sind meist asymptomatisch oder zeigen initial nur uncharakteristische Beschwerden. Die Diagnose erfolgt durch bildgebende Maßnahmen (US, CT, MRT) und den serologischen Nachweis von Echinococcus-Antikörpern. Die Therapie der zystischen Echinokokkose orientiert sich an der biologischen Aktivität der Zysten entsprechend der Bildgebung mit dem Ziel einer Sanierung aller vitalen Elemente durch Drainage oder Resektion der Zysten mit gleichzeitiger medikamentöser Albendazoltherapie oder alleiniger medikamentöser Therapie. Die alveoläre Echinokokkose kann nur durch radikale operative Therapie geheilt werden. Eine zusätzliche Albendazolmedikation verringert die Rezidivhäufigkeit oder kommt bei nicht operablen Fällen zur Überlebenszeitverlängerung als alleinige Maßnahme zum Einsatz. Die in Deutschland insgesamt seltene Erkrankung sollte interdisziplinär in erfahrenen Zentren behandelt werden
