Zusammenfassung
Innerhalb der Kardiologie gilt die Digitalisierung heute als eines der Hauptfelder des medizinischen Fortschritts. Unter allen eHealth-Anwendungen innerhalb des Fachgebietes ist die Telemedizin bei Herzinsuffizienz am weitesten entwickelt in Hinblick auf Umsetzbarkeit und Evidenz. In der Betreuung von Herzinsuffizienzpatienten wird die weiterhin bestimmende Präsenzbetreuung in Praxis und Krankenhaus ergänzt durch eine telemedizinische Mitbetreuung. Für die dazu notwendige tägliche Diagnostik von Vitalparametern aus der Häuslichkeit des Patienten kommen nichtinvasive und implantierte Heimmessgeräte zum Einsatz. In drei Landmarkstudien konnte eine Überlegenheit dieser Intervention bezüglich Mortalität und Morbidität gegenüber Patienten ohne telemedizinische Mitbetreuung gezeigt werden (Abraham et al., Lancet 377:658–666, 2011; Hindricks et al., Lancet 384:583–590, 2014; Koehler et al. 2018; Lancet 392(10152):1047–1057).
