Zusammenfassung
Humane induzierte pluripotente Stammzellen (hiPS-Zellen) können patienten- und krankheitsspezifisch aus nahezu jeder Zelle des Körpers erzeugt werden und besitzen Eigenschaften, die eigentlich nur Zellen aus einem Embryo besitzen. Ihre einfache Herstellung und ihre hohe Verfügbarkeit ermöglichen eine Vielzahl an neuartigen zellbasierten Therapiemethoden und therapeutischen Einsatzmöglichkeiten. Der gesamte Arbeitsablauf, der für eine klinische Anwendung von hiPS-Zellen nötig ist, lässt sich in die Gewinnung (Generierung), die Vervielfältigung (Expansion), die Spezialisierung (Differenzierung), die funktionsverlustfreie Lagerung (Kryokonserierung), die Charakterisierung (Qualitätskontrolle) sowie die finale Applikation (Zelltherapie) der hiPS-Zellen bzw. der hiPS-Zell-basierten Produkte unterteilen. Jeder dieser Schritte besitzt einen entscheidenden Einfluss auf die Qualität und Funktionalität des resultierenden hiPS-Zell-basierten Therapieproduktes. Daher werden in diesem Beitrag die naturwissenschaftlichen Grundlagen und die Umsetzung der genannten Prozessschritte mit ihren Chancen und Risiken nach aktuellem Stand der Technik erläutert sowie Ausblicke auf zukünftige praktische Fragestellungen gegeben.

Unter Adhäsion (Verb: adhärieren) versteht man die Anheftung von Zellen an andere Zellen oder an ein Substrat über Zelladhäsionsmoleküle.

Allele = Varianten eines Gens, die durch Mutationen ineinander überführt werden können. Bei diploiden Zellen liegen für jedes Gen zwei Allele vor.
-
Bei allogenen Therapien sind Spender und Empfänger verschiedene Personen.
-
Apoptose bezeichnet den über Genexpression von der Zelle gesteuerten Zelltod; wird auch als „programmierter Zelltod“ oder „Zell-Selbstmord“ bezeichnet.

Bei der autologen Therapie sind Spender und Empfänger die gleiche Person, was den großen Vorteil besitzt, dass es theoretisch zu keiner immunologischen Abstoßungsreaktion kommt.

Eine Clusteranalyse ist eine Methode zur Suche nach Ähnlichkeitsstrukturen in Datensätzen. Die gefundenen ähnlichen Daten werden in Gruppen zusammengefasst und als Cluster bezeichnet. Das Cytoplasma füllt das Innere jeder Zelle aus. Es besteht aus einer flüssigen Substanz (Cytosol), dem Zellskelett (Cytoskelett) und den Zellorganellen.

Differenzierung (von lateinisch differre = sich unterscheiden) bezeichnet den Entwicklungsprozess einer Zelle von einem weniger in einen stärker spezialisierten Zustand. Dieser Prozess ist in der Regel irreversibel. Dabei entstehen strukturell und funktionell unterschiedliche, auf bestimmte Aufgaben angepasste Zellen.

Als diploid wird eine Zelle bezeichnet, wenn ihr Chromosomensatz doppelt vorhanden ist, das hei-ßt jedes Chromosom liegt also zweimal vor. Körperzellen des Menschen sind diploid.

DNA (deoxyribonucleic acid) = Desoxyribonukleinsäure (s. auch Genom, Expression)

Die DNA-Methylierung ist ein epigenetischer Mechanismus, bei dem Methylgruppen an Nukleotide gebunden werden (meist wird Cytosin in 5-Methylcytosin umgewandelt). Das DNA-Methylierungsmuster ist zelltypspezifisch und mitverantwortlich dafür, dass die Zelltyp-spezifischen Gene exprimiert werden.

Endozytose ist die Aufnahme von Flüssigkeit oder Partikeln aus der Umgebung der Zelle durch Einstülpung der Zellmembran

Unter Epigenetik werden alle auf Tochterzellen vererbbaren Modifikationen der Genexpression zusammengefasst, die nicht auf einer Veränderung der DNA-Sequenz basieren. Zu den bedeutendsten epigenetischen Mechanismen zählen DNA-Methylierungen und die Komposition bzw. Modifikation von Histonen.

Epithel ist das Deck- oder Drüsengewebe, das aus ein- oder mehrlagigen Zellschichten bestehen kann und die inneren und äußeren Körperoberflächen bedeckt.

Bei der Exomsequenzierung wird nicht das vollständige Genom, sondern nur die Gesamtheit der für Proteine kodierenden Bereiche (Exons) untersucht.

Expression (Verb: exprimieren) ist der Vorgang, bei dem die genetische Information umgesetzt wird, das heißt ein entsprechendes Protein synthetisiert wird. Zunächst wird die DNA-Sequenz des Gens in eine RNA-Sequenz (mRNA) umgeschrieben (Transkription). Anschließend wird diese in die entsprechende Aminosäuresequenz translatiert.

Die extrazelluläre Matrix ist die Gesamtheit aller Makromoleküle, die außerhalb der Zellen die Zwischenräume ausfüllt

Feederzellen = Fütterzellen, die in der Zellkultur eingesetzt werden, da sie Stoffe ins Medium sekretieren, welche das Wachstum anderer Zellen fördern, und eine extrazelluläre Matrix produzieren, an die andere Zellen anhaften können

Fibroblast = im Bindegewebe vorkommender Zelltyp mesenchymaler Herkunft

Gameten = Geschlechtszellen oder Keimzellen. Zwei Gameten mit haploidem Chromosomensatz vereinigen sich bei der geschlechtlichen Fortpflanzung (beim Menschen Eizelle und Spermium).

Ein Gen ist ein definierter Abschnitt im Genom und die kleinste Funktionseinheit des Erbgutes. Die Expression von Genen führt zur Bildung von mRNA und Proteinen.

Unter dem Genom versteht man die Gesamtheit der vererbbaren Informationen, welche beim Menschen in der DNA-Sequenz gespeichert und in Abschnitten (Genen) organisiert ist.

Unter der Genomintegrität versteht man die Makellosigkeit des Genoms, die gegeben ist, wenn keine Veränderungen der DNA-Sequenz vorliegen.

Unter dem Somatischen Zellkerntransfer versteht man die Übertragung des Zellkerns einer differenzierten Körperzelle in eine entkernte Eizelle, was in der Umprogrammierung des Zellkerns resultiert. Es entsteht ein Klon.

Als Telomere werden die Enden jedes Chromosoms bezeichnet. Sie bestehen aus repetitiven DNA-Sequenzen und assoziierten Proteinen. Die Telomere werden vom Enzym Telomerase erneuert. Dieses ist in menschlichen Körperzellen nicht aktiv, weswegen sich die Telomere mit jeder Zellteilung verkürzen.

Ein Teratom ist ein Tumor, der eine Mischung von Zellen aus allen drei Keimblättern enthält. Unter natürlichen Bedingungen können Teratome beispielsweise aus Keimzellen entstehen. Pluripotente Zellen bilden Teratome nachdem sie immunsuppremierten Mäusen injiziert wurden.

Unter Transdifferenzierung versteht man die Umwandlung einer differenzierten Körperzelle in einen anderen Zelltyp ohne pluripotente oder multipotente Zwischenstufe.

Ein Transkriptionsfaktor ist ein Protein, das für den Start der Transkription notwendig ist.

Bei der Translation wird die mRNA mit Hilfe von Ribosomen und von transferRNAs (Übertragungs-RNA, tRNA) in die entsprechende Aminosäuresequenz übersetzt, wobei jeweils drei Nukleotide für eine Aminosäure kodieren. Die Aminosäurekette erhält schließlich durch Faltung die native dreidimensionale Proteinstruktur.
