Zusammenfassung

Die systemisch zirkulierende amidolytische Thrombin-Aktivität (F2a···α2M) ist ein Maß für die akute oder chronische Thrombin-Generierung in vivo. 100% der Norm sind 5.5 mIU/ml, der Normbereich liegt bei 80-120% (normal intravascular coagulation = NIC). 121-150% bedeutet Vorphase der pathologischen intravasculären Gerinnung (PIC-0), 151-200% bedeutet typische Phase der pathologischen intravasculären Gerinnung (PIC-1), >200% bedeutet Verbrauchsphase der pathologischen intravasculären Gerinnung (PIC-2; oft mit INR > 1.15). Supra-1-molare finale Konzentrationen an Arginin machen den Test spezifisch für Thrombin, indem sie Kallikrein-Aktivität komplett inhibieren und Antithrombin-1 (nascent fibrin) depolymerisieren. Basierend auf diesem Testprinzip wurden ultra-spezifische F10a/F2a-Generierungsteste abgeleitet, so der recalcified coagulation activity assay (RECA), der intrinsic coagulation activity assay (INCA), oder der extrinsic coagulation activity assay (EXCA).
