Zusammenfassung
Eine rein diagnostische perkutane transhepatische Cholangiographie (PTC) wird angesichts der Weiterentwicklung der schnittbildgebenden Verfahren heutzutage kaum noch durchgeführt. Die perkutane transhepatische Cholangiodrainage (PTCD) kommt als Palliativtherapie beim malignen Verschlussikterus zum Einsatz, wenn eine chirurgische Therapie aufgrund eines fortgeschrittenen Tumorwachstums onkologisch nicht mehr sinnvoll ist und eine endoskopische Therapie nicht möglich ist. Dies ist z. B. bei Patienten mit endoskopisch nicht rekanalisierbarer Tumorstenose oder bei Z. n. Whipple-Operation der Fall. Auch bei Patienten mit benignem Verschlussikterus kann eine PTCD durchgeführt werden, insbesondere dann, wenn eine endoskopische Rekanalisation aus anatomischen Gründen nicht durchführbar ist oder nicht gelingt. Zudem kann eine Galleableitung über eine PTCD präoperativ bei Patienten mit kurativer Operationsindikation notwendig sein. Diese wird dann meist nur als externe Ableitung durchgeführt.
Ein weiteres Einsatzgebiet der PTCD stellt die Therapie postoperativer Komplikationen dar, beispielsweise bei Patienten mit einer Insuffizienz der biliodigestiven Anastomose oder einer ischämischen Cholangiopathie nach Lebertransplantation.
