Abstract
For many patients with several years of seizure freedom, it is appropriate to consider whether antiseizure medications (ASMs) can be weaned. There are many reasons why patients are interested in stop** ASMs: long-term and short-term side effects, family planning, potential interactions with other treatments, and a desire to minimize medications taken. Though there are specific factors associated with a higher risk of seizure recurrence, long duration of seizures, structural lesions, continued epileptiform discharges on EEGs, there is not a single test that can ensure seizure freedom after ASM discontinuation. Clinical history and ancillary testing all need to be carefully considered to determine the risk of seizure recurrence in order to help the patient make an informed decision regarding ASM discontinuation.
