Rc Fistula for Palate Re-repair and Pharyngeal Flap

Abstract

This patient was operated for right-sided unilateral complete cleft palate but developed a fistula in the lateral portion of the soft palate on right side. When presented to us, he had a very short palate with severe nasal air emission. There is no intra-oral pressure to produce consonants. The voice quality is severely compromised to be of Glottal type with severe type of hypernasality. As per Pakistan Comprehensive Fistula Classification, this patient has right lateral fistula Rc, with S3 & V3

Plan of Management: Turn-in flaps will be used for nasal layer closure, while oral layer closure will need some extra tissues from the soft palate area. But as the palate is already very short with very poor movements, pharyngeal flap will be added to provide extra tissues and to lengthen the palate.
