Mangelernährung, Sarkopenie, Kachexie: Ursachen, Diagnostik, Therapie

Zusammenfassung

Die Nahrungsaufnahme ist bei Patienten mit fortgeschrittener Tumorerkrankung häufig reduziert und unzureichend. Der daraus resultierende Gewichts- und Zellverlust ist mit einer eingeschränkten Prognose, verringerter Therapietoleranz und mit verminderter Lebensqualität assoziiert. Parallel entwickelt sich oft – in individuell unterschiedlichem Ausmaß und mit Fortschreiten der Erkrankung zunehmend – eine tumorassoziierte systemische Inflammationsreaktion, offensichtlich ausgelöst durch Interaktionen zwischen Tumorstroma und malignen Zellen. Diese Veränderungen beeinflussen alle wesentlichen Stoffwechselwege und resultieren ganz allgemein in anaboler Resistenz und Überwiegen kataboler Prozesse. Eine Stoffwechsel- und Ernährungstherapie soll einen Verlust an Körperzellmasse verlangsamen oder umkehren und so den Ernährungszustand stabilisieren oder sogar normalisieren. Für den Patienten zielt dies auf eine Verbesserung der Lebensqualität, der Mobilität und körperlichen Unabhängigkeit, der Verträglichkeit antitumoraler Therapien sowie des weiteren Erkrankungsverlaufs. Die Indikationen für eine Ernährungstherapie entsprechen prinzipiell denen bei benignen Erkrankungen. Wegen des erwarteten hohen Risikos für eine Mangelernährung wird allerdings empfohlen, bereits beginnend mit der Tumordiagnose regelmäßig und möglichst standardisiert nach Anzeichen von Ernährungsstörungen zu suchen und beim Nachweis eines Ernährungsrisikos umgehend eine Ernährungsbehandlung einzuleiten.
