Samenvatting
Bisfosfonaten, geneesmiddelen die de botresorptie remmen, hebben een vervelende bijwerking: kaakbotnecrose. Deze botnecrose bij bisfosfonaatgebruik volgt meestal op een ingreep in de mondholte, zoals extractie van een gebitselement. Voor de tandarts is het van belang te denken aan osteonecrose als de patiënt bisfosfonaten gebruikt en zich presenteert met een blootliggend, niet-genezend stuk kaakbot. Kaakbotnecrose komt vooral voor bij de intraveneuze behandeling met bisfosfonaten; de prevalentie bedraagt 4 tot 10 procent. Bij orale behandeling met bisfosfonaten is de prevalentie beduidend lager. De gepresenteerde casuïstiek illustreert dat bisfosfonaten gerelateerde osteonecrose van kaakbot lastig te behandelen is en dat een conservatieve benadering de voorkeur heeft. De behandeling bestaat uit chloorhexidine mondspoelmiddel, zo nodig antibiotische ondersteuning en beperkte debridement. Tijdens behandeling met bisfosfonaten zijn een optimale mondhygiëne, regelmatige tandheelkundige controle en behandeling en bij tandelozen een goed passende gebitsprothese belangrijk. Invasieve ingrepen in de mondholte dienen zo veel mogelijk vermeden te worden.
