Zusammenfassung
Bisherige klinische Erfahrungen mit Etoposid haben gezeigt, daß so verschiedenartige Neoplasien wie Bronchialkarzinome, Hodenkarzinome, Non-Hodgkin-Lymphome oder monozytäre Leukämien bemerkenswert gut, andere Tumorarten aber deutlich weniger auf dieses Podophyllotoxinderivat ansprechen (2, 8, 9, 24). Die Ursachen dieses unterschiedlichen Verhaltens sind einerseits im Wirkungsmechanismus des Präparates, andererseits in spezifischen Eigenschaften der jeweiligen Tumorzellpopulationen zu suchen. Als Interphasenhemmstoff greift Etoposid in die RNS- und DNS- sowie die prämitotische Proteinsynthese ein (1, 3, 7) und beeinflußt besonders jene Zellen, in denen diese Syntheseprozesse ablaufen. Daneben hängt seine Wirksamkeit unter anderem von der Permeabilität der Tumorzellmembranen, der Metabolisierung innerhalb der Zellen und den Interaktionen mit energieabhängigen Stoffwechselprozessen ab. — Für den gezielten klinischen Einsatz des Präparates wäre daher eine genauere Kenntnis der genannten Faktoren nötig. Da dies im Einzelfall praktisch unmöglich ist, muß sich die Gabe des Präparates fast ausschließlich nach klinischen Erfahrungswerten richten. Um dennoch, über das empirische Wissen hinaus, den Einsatz steuern und die klinische Wirkung vorhersagen zu können, haben wir den Effekt von Etoposid in einem In-vitro-Testsystem untersucht.
