Zusammenfassung

Der Hoden hat 2 wichtige Funktionen:eine endokrine, d.h. die Bereitstellung ausreichender Mengen des männlichen Sexualhormons Testosteron, und eine exokrine, d.h. die Bildung befruchtungsfähiger Spermien für eine erfolgreiche Fortpflanzung. Beeinträchtigungen derTestosteronwirkung und/oder der Fertilität des Mannes können auf Störungen im Bereich des Hypothalamus, der Hypophyse und der Hoden oder auf posttestikulären Störungen beruhen. Nach entsprechender Diagnostik können heute sehr effektive rationale Therapieverfahren sowohl für die endokrine als auch die exokrine Funktion angewandt werden.

Eine Gynäkomastie des Mannes ist kein eigenständiges Krankheitsbild, sondern meist ein Symptom einer gestörten Androgen-Östrogen-Balance.Vor einerTherapie sind eine ausführliche Anamnese und Untersuchung erforderlich, da u.a. auch endokrin aktive Hodentumoren eine Gynäkomastie bedingen können.

Hodentumoren sind häufige Malignome junger Männer im fortpflanzungsfähigen Alter. Die Therapie erfolgt stadienadaptiert unter Zusammenarbeit von Onkologen, Urologen und Strahlentherapeuten. Seit Einführung des Medikamentes Cisplatin in dieTherapie der Keimzelltumoren handelt es sich um chemosensible Neoplasien mit prinzipiell günstiger Prognose.
