Probleme der Keratoplastik

Zusammenfassung

Anatomische und funktionelle Ergebnisse der Keratoplastik stehen häufig nicht im Einklang. Der postoperative Astigmatismus wirkt optisch meist sehr störend. Über die Ursachen des Astigmatismus bei 500 Patienten mit perforierender Keratoplastik wird nach Datenverarbeitung mit breit angelegtem Schlüssel und Auswertung durch einen Computer berichtet.

Störungen im Heilverlauf, unvollkommene Operationstechnik, Transplantatgröße, Nahtform und Zeitpunkt der Nahtentfernung sowie die Lage des Transplantates können verantwortlich sein. Nur bei allseits scharfrandigen, senkrechten und gleichmäägen Schnitträndern an Transplantat und Wirtscornea ist eine ideale Wundadaptation und spannungsfreie Einheilung gewährleistet. Deshalb muß auch eine exakte Technik für die Gewinnung der Spenderscheibe, die ein wesentlicher Störfaktor sein kann, die gleiche Beachtung finden. Hierzu Mitteilung experimenteller Ergebnisse.

Abschließend wird über die bisher durchgeführten Histokompatibilitätstestungen und ihre Ergebnisse bei der Hornhauttransplantation beriehtet.

Summary

Frequently there is a discrepancy of anatomical and functional results of corneal grafts. One of the most disturbing problems is the postoperative astigmatism. A computer-analysis of 500 patients with perforating keratoplasty was done to evaluate the causes of the postoperative astigmatism.

Most frequent causes were disturbance in the postoperative period, failure in the meticulous details of technic, size of the graft. Technic and time of removal of the sutures as well as the situation of the graft are responsible for the degree of the postoperative astigmatism. Sharp edged, vertical and regular outlined woundmargins of the graft and host are essential for a good adaptation and healing without tension. Therefore it is of great importance to follow a meticulous technic in receiving the donor graft. This technic is experimentally investigated.

Finally it is reported about the results of histokompatibility-testing in keratoplasty.

Résumé

Les résultats anatomiques et fonctionels des kératoplasties sont le plus souvent pas concordant. L’astigmatisme post-opératoire est très génant pour la vision. Les causes de cet astigmatisme chez 500 patients ayant subi une greffe perforante sont étudiés selon des normes statistiques avec un moyenneur et un ordinateur.

Les suites post-opératoires, une technique opératoire déficiente, le type des sutures et le temps d’enlèvement des fils, ainsi que le siège du greffon, peuvent être responsables de cet astigmatisme. Seule une bonne coaptation du greffon avec un parfait parallélisme des surfaces épithéliales avec celle de la cornée réceptrice peut éviter autant que possible l’apparition fracheuse de cet astigmatisme. C’est pourquoi la technique de prélèvement du greffon, qui est aussi un facteur non moins important, mérite une très grande attention. L’auteur fait part de ses résultats expérimentaux et en conclue avec des données nouvelles obtenues après étude de l’histocompatibilité dans les greffes perforantes corneennes.
