Zusammenfassung

Wir kennen keine intensivmedizinische Arbeit über Spontanatmung, druckunterstützte Spontanatmung, Weaning oder Beatmung, in der die Kenntnis der mechanischen Eigenschaften der Lunge und des respiratorischen Systems zwecks klinischer Beurteilung von Zustand und Verlauf nicht als wichtig eingestuft würde (Behrakis et al. 1983; Gattinoni et al. 1984; Bates et al. 1985; Gottfried et al. 1985; Rossi et al. 1985; Milic-Emili et al. 1987; Bernasconi et al. 1988; Broseghini et al. 1988; Bertschmann et al. 1989; D’Angelo et al. 1989; Eissa et al. 1991; Eberhard et al. 1992; Guttmann et al. 1992; Tantucci et al. 1992). Wir teilen diese Meinung; für besonders wichtig halten wir die Kenntnis der mechanischen Zusammenhänge, wenn der Atemwegsdruck — wie bei der druckunterstützten Spontanatmung — in therapeutischer Absicht manipuliert wird (Dittmann et al. 1977; Anderes et al. 1979; Maclntyre 1986; Brochard et al. 1987; Brochard et al. 1989; Hursh et al. 1991). Aus diesem Grund soll im folgenden die Situation der intubierten, druckunterstützt spontanatmenden Patienten genauer untersucht werden.
