Zusammenfassung
Der Verlauf von Patienten, die wegen terminaler Leberzirrhose auf dem Boden von Viruserkrankungen orthotop lebertransplantiert worden sind, wird oft durch Reinfektionen des Transplantates mit dem Virus kompliziert [1]. Wird präoperativ eine ausgeprägte Virämie und/oder hohe Replikation des Hepatitisvirus festgestellt, gilt das Reinfektionsrisiko als erhöht [2]. Daher wurde 1994 im Rahmen eines Consensus-Reports empfohlen, Patienten mit diesen Risikofaktoren nicht zur Transplantation zu melden [3]. Neuere Studien haben den Grad der Übereinstimmung des HLA-Komplexes zwischen Spender und Empfänger („HLA-match“) in Zusammenhang mit der Transplantatreinfektion bei Virushepatitiden gebracht [4, 51. In der vorliegenden Studie untersuchten wir den Einfluß von HLA- Übereinstimmungen auf die Prognose von Patienten, die wegen einer terminalen Hepatitis-B-Zirhose transplantiert wurden.
