Summary
Once intrahepatic adenocarcinoma without primary is diagnosed, differentiation between cholangiocarcinoma (CCC) and metastatic adenocarcinomas of unknown primary can be difficult. To examine the influence of CK 7 on diagnosis and prognosis, we investigated the course of 53 patients. After conventional histological classification (CCC = 74%, n = 39; unknown = 26%, n = 14), the expression of CK 7 was evaluated (positive = 83%, n = 44; negative 17%, n = 9). Comparing the postoperative course of both groups, we found no significant difference in survival. The 2-year-survival was 30%. A secondary tumor manifestation could not be documented. The preoperative management has shown strong resistance to exclude an extrahepatic primary. CK 7 increases the sensitivity of histological diagnosis of CCC without prognostic relevance.
Zusammenfassung
Trotz neuer immunhistochemischer Verfahren fällt beim intrahepati- schen Adenocarcinom die Differenzierung zwischen Cholangiocellulärem Carzinom (CCC) und Metastase ohne Primariusnachweis schwer. Die Bedeutung von CK 7 für die Diagnose und Prognose wird bei 53 Patienten überprüft. Alle Histologien wurden konventionell klassifiziert und dann retrospektiv auf CK 7-Expression nachuntersucht (positiv = 83%, n = 44; negativ 17%, n = 9). Die mittleren Überlebenszeiten der einzelnen Gruppen unterschieden sich nicht signifikant. Das 2-Jahres-Gesamtüberleben betrug 3o% ohne das ein extrahepatischer Primarius auftrat. Das präoperative Management bietet eine hohe diagnostische Sicherheit zum Ausschluß extrahepatischer Primärtumoren. CK 7 erhöht die Sensitivität der Diagnose des CCC ohne signifikante prognostische Relevanz.
